Designed to help managers provide feedback to employees about performance, impact, and areas to improve, this review is typically completed on a six month cadence. The process helps both employees and managers recognize areas to improve, celebrate successes, and inform personal development goals. By tracking performance, a manager and business can also understand when an employee’s role has changed and compensation adjustments are necessary. 

How to use it
  • Copy and save or download the questionnaire.

  • Take time to really reflect on the employee’s progress, achievements and challenges.

  • Complete the questionnaire and meet 1-on-1 to offer constructive feedback.

  • Work with your employee to set personal goals that align with the company’s goals.
